Human-in-the-loop (HITL) is a pattern where AI systems operate autonomously for routine operations but pause for human approval on high-stakes decisions. This creates a hybrid model that captures the efficiency benefits of automation while maintaining human oversight where it matters most.
What Is Human-in-the-Loop AI?
Human-in-the-loop AI refers to systems where humans are integrated into the AI decision and action cycle. Rather than operating fully autonomously, the AI pauses at critical points to request human input, approval, or oversight. The human reviews the proposed action, considers context the AI might not have access to, and makes the final decision.
In the context of AI runtime governance, HITL manifests as approval workflows. When an AI agent attempts an action that triggers an approval requirement, the action is paused. A notification is sent to designated approvers with full context about what the agent wants to do and why. The approver can approve, reject, or request modifications before the action proceeds.
This pattern differs from post-hoc review where humans audit AI actions after they happen. HITL is preventative: actions do not execute until approved. This is critical for irreversible actions where post-hoc review cannot undo damage.

How It Works Technically
A HITL approval workflow involves several components working together:
Escalation Triggers
Policies define which actions require approval. Triggers can be based on action type, parameter values, resource sensitivity, risk score, or combinations of factors. When an action matches an escalation trigger, the governance layer pauses execution and initiates the approval workflow.
Context Packaging
The approval request packages all relevant context for the human reviewer. This includes what action is being requested, what parameters are involved, which agent is requesting it, why the escalation was triggered, and relevant history. Good context packaging enables informed decisions without requiring the approver to investigate independently.
Notification and Routing
Approval requests are routed to appropriate reviewers based on configurable rules. Different action types might route to different teams. High-risk actions might require multiple approvers. Notifications are delivered through configured channels: Slack, email, SMS, or in-app notifications.
Decision Capture
When the approver makes a decision, the system captures the outcome, any comments or conditions, and the approver's identity. This creates an audit trail showing who approved what and when. If approved, the action proceeds. If rejected, the action is blocked and the agent receives notification of the rejection.
Timeout Handling
Approval workflows must handle cases where no response is received. Policies can define timeout behavior: automatic rejection after a certain period, escalation to additional approvers, or automatic approval for certain low-risk items. Timeout policies balance operational continuity with security requirements.
Example Scenario
A financial services company deploys an AI agent to process customer refund requests. The agent can access order history, validate refund eligibility, and initiate refunds. The company wants automation for standard refunds but human oversight for exceptions.
Approval Policy:
• Refunds < $50: Automatic approval
• Refunds $50-$200: Single manager approval
• Refunds > $200: Finance team approval
• Repeat refunds (same customer, 30 days): Always require approval
A customer requests a $75 refund for a defective product. The agent validates the order, confirms the return window, and initiates the refund. Because $75 falls in the $50-$200 range, the action pauses and the customer service manager receives a Slack notification with full context.
The manager reviews the request, sees that the product has a known defect, and approves the refund. The agent receives approval and completes the refund. The entire interaction is logged: the customer request, the agent's evaluation, the escalation trigger, the manager's approval, and the completed refund.
